Which Patients Are Eligible?

Any current Medicare cardholder may be eligible — particularly those at risk of medication misadventure.

Patients do NOT need to be on 5 or more medications to be eligible.

Significant changes to medication regimen
Recent hospital discharge or frequent readmissions
High-risk medications (opioids, anticoagulants, insulin, psychotropics, NSAIDs)
Medications requiring monitoring (warfarin, digoxin, lithium, antiepileptics)
Complex medication regimen or polypharmacy
Suspected adverse drug reaction or interaction
Suspected non-adherence or difficulty managing medications
Functional issues: frailty, frequent falls, swallowing difficulty, renal/hepatic impairment
Cognitive difficulties, dementia, language/literacy barriers
Attending multiple doctors or specialists
Abnormal pathology results
Prescribing cascade (one medicine treating side effects of another)

Patients are eligible for an HMR every 12 months, or earlier if there is a significant change in their medical condition or medication regimen.

Identify Eligible Patient

Assess patient's eligibility and obtain consent.

Send Referral

Send via email or fax with relevant clinical information (reason for review, recent pathology if applicable).

We Contact the Patient

We reach out to the patient within 48 hours to schedule the home visit.

You Receive the Report

A comprehensive HMR report with recommendations is delivered to you within 2 weeks. You then develop the Medication Management Plan (MMP) with the patient and submit the MBS Item 900 claim.
